Being diagnosed with diabetes doesn't have to mean the end of life, as you know it. Let the Diabetes Self Management program help you cope. From education to support. We are dedicated to improving the quality of life for individuals with diabetes living in our community. We serve by providing high quality individualized diabetes self management training with a personal touch.

Why is it important to seek a Diabetes Educator?
Diabetes is a complex disease that requires daily self-management. A diabetes educator is a health care professional who specializes in the care and education of people with diabetes.

Why should I choose The Diabetes Self-Management Program?
Our classes are small and tailored to meet your individual needs. Each class has a maximum of six participants. We encourage you to bring a family member for support and education. Our curriculum is an accredited program through the American Diabetes Association (ADA).

What to expect at the first visit?
During your first visit, you will learn the basics of diabetes and nutrition in order to develop a self-management plan. We will discuss:

   * Types and causes of diabetes
   * High blood glucose (sugar)
   * Low blood glucose
   * Self-monitoring of blood glucose levels
   * Diabetes medication
   * Insulin administration
   * Planning healthy meals
   * Counting carbohydrates
   * Reading food labels
   * Sick day guidelines

What to expect during follow up visits?
Follow up consultations are classes that are tailored to your particular needs and will include

   * Principles of self management
   * Continued diabetes education
   * Reducing risk for complications
   * The affects of diabetes on your body
   * Problem solving
   * Blood sugar log review
   * Food diary review
   * Dealing with stress
   *

What resources are available to me?
   * Diabetes Educator
   * Registered Dietitian
   * BI Monthly Diabetes Support Group meets the 2nd Thursday of the
     month in the first floor conference room in the hospital

How can I take part in the program?
Diabetes Self-Management training program is an outpatient program. A doctor’s referral is required to participate.

Will my insurance cover the program?
A physician’s referral is required to participate and most insurance plans will cover diabetes education. Please check prior to your appointment for insurance coverage or pre-authorization requirement.

How do I get to the Diabetes Self-Management Program?
We are located on the first floor of Southside Community Hospital. You will need to register in the registration department located on the first floor of the hospital.
